Screening Form

At Tamarack, we are committed to doing our best to ensure the health and safety of our employees, clients, and the public. We are currently implementing all public health guidelines in our operations in order to help reduce the spread of COVID-19. To this end, we require that all visitors complete the following form prior to visiting any Tamarack property. The information collected through this screening will be used to enhance our health and safety measures and allow us to take all necessary precautions. All information will be kept confidential and will be used only for the purposes specified here.

If you received a COVID-19 vaccination in the last 48 hours and are experiencing mild headache, fatigue, muscle aches, and/or joint pain that only began after vaccination, select “No.” Yes No

2. Has a doctor, health care provider, or public health unit told you that you should currently be isolating (staying at home)? * Yes No

3. Do you live with someone who has been told by a doctor, health care provider, or public health unit that they should currently be isolating? If you are fully vaccinated, select “No.” *
If the individual experiencing symptoms received a COVID-19 vaccination in the last 48 hours and is experiencing mild headache, fatigue, muscle aches, and/or joint pain that only began after vaccination, select “No.” Yes No

If you have answered yes to any of the screening questions above, we ask that you do not enter our sites at this time. We are happy to reschedule your visit at a later date and encourage you to stay home and self-isolate as per the recommendation of our public health officials.
